Physician COVID Vaccine Information

Nebraska DHHS
Physicians can submit a Vaccine For Children (VFC)/COVID Provider Agreement Form to accept, manage, track and administer COVID vaccine. Requirements for doing this include cold chain storage, minimum staffing is at 100, use of NESIIS for registration/data entry, etc. If interested, you can contact DHHS at 402-471-6423 or 800-798-1696 or by email: dhhs.immunization@nebraska.gov

Douglas County Health Department
If you don’t meet the requirements to be a VFC/COVID vaccine provider and you aren’t on staff at a medical system, you will need to get the vaccine from the Douglas County Health Department when it becomes available. You can bring it to your practice and you can administer it to your staff (DCHD will oversee the cold chain storage of the vaccine and assist with registration/data entry) or they can administer the vaccines to your staff for you. Vaccination Survey for Non-VFC Practices
The Metro Omaha Medical Society, in partnership with the Douglas County Health Department, is working to identify non-VFC practices and gather information regarding their staffing and vaccination practices.  This information will be used in DCHD vaccination planning, including the COVID-19 vaccine.

Sarpy/Cass Health Department
Sarpy/Cass Health Department (SCHD) is gathering information to assist in finalizing plans to coordinate vaccination efforts focusing on healthcare workers within the jurisdiction.

The purpose of this survey is to assess the number of healthcare staff working in your clinic/practice who you anticipate will be vaccinated for COVID-19. Because of the logistical issues and requirements of the Nebraska COVID-19 vaccine response team, they need some specific information about your facility. They will be using this information to finalize plans to ensure your staff have access to the vaccine.

If your clinic/practice has more than one location, please enter a separate survey for each location.
